Case Management Services
HCF services also include case management which promotes maximum independence and successful inclusion into the community. The main goal is to maximize individuals’ freedom of choice, control, integration and individualization. Accountability and continuity of services and supports are available to individuals and their families for as long as services are needed.
Heartstrings has 3 Case Managers on staff with a vast knowledge of the state programs and the state benefits that children and adults with special needs are eligible for. If you are interested in learning more about this service, please give us a call!
